NAD+ Signature Membership Service Agreement
This Agreement is made between the undersigned patient and Medical Health and Wellness regarding participation in the NAD+ Signature Membership.
1. Services Provided
Provider agrees to supply NAD+ (Nicotinamide Adenine Dinucleotide) wellness products and/or support services, including self-injection guidance, member education, and periodic follow-up, as outlined in the membership program.
2. Member Responsibilities
Member agrees to:

3. Payments & Renewals
Membership is billed monthly at the rate of $185. Automatic billing continues until canceled. No refunds will be given for unused products or time.
4. Medical Disclaimer
This program is not a substitute for medical care. Member acknowledges that NAD+ is a wellness therapy and agrees to consult their healthcare provider as needed. Provider does not diagnose, treat, or guarantee medical outcomes.
5. Termination
Either party may cancel the membership after to 90 day commitment. Access to services ends at the close of the billing cycle.

By enrolling, the patient confirms they have read and agree to the terms above.
